Screening Forms

For all new patients we require a medical info and consent form to be completed, the link to this will be on the confirmation email you receive when you are booked in.


Arrival to the clinic – when you arrive at the clinic, please take a seat in the upstairs waiting area. If you are unable to get upstairs due to disability or injury, please let us know if you can before you arrive and we will meet you downstairs and direct you to our accessible consultation room.

PPE –  you are no longer required to wear a face covering when attending an appointment. 

We have installed hand sanitiser dispensers throughout the clinic for your use so please use them freely.

Cleaning – we implement a strict cleaning regime between each patient to ensure best practice is adhered to. Each treatment room is thoroughly cleaned after each session. Every piece of gym equipment, is cleaned after every session so you can be assured when you arrive to the clinic, the standards of cleanliness are as high as possible.

FAQs

We aim to provide you with an excellent service from start to finish. If you have any specific questions regarding your appointment or what to expect, please contact us and we’ll be more than happy to answer any questions you might have. Below are answers to some of the most common questions that people ask us; if your query isn’t covered then please don’t hesitate to contact us.

What should I bring?

Ideally a pair of shorts, especially for any lower body injuries. Any relevant footwear would also be helpful, i.e. running trainers, football boots, running spikes etc. For back/neck/shoulder injuries, please bring a top which will allow access to the area, or which you don’t mind taking off.

If you are attending for any conditioning work then a bottle of water would also be recommended.

If you’ve had any previous medical treatment, please bring along any relevant paperwork, consultant’s letters, medications lists etc.

Can I claim on my medical insurance to see you?

Some medical and healthcare cash plans will cover us but some will not. Please check your policy and get in touch with us if you need to check.

Is there anywhere to park?

There is free parking available outside the clinic. Please look for the parking bays with our logo on, if they are all taken, there is free parking on George Cayley Drive.

Are there disabled accessible facilities?

Our clinic is based across two floors with consultation rooms on the ground and first floor. There is no lift so if you are unable to use the stairs, please let us know ahead of your appointment and we will arrange to see you in the accessible downstairs consultation room. Please inform us if you have any special requirements and we’ll always do our best to cater for them.

How can I pay?

We accept cash, BACS and all major credit and debit cards.
